<p><strong>RN Hospice Case Manager – Full-Time | $75,000–$80,000&nbsp;</strong></p>

<p>We are seeking a compassionate and experienced <strong>Hospice RN Case Manager</strong> to join our team in a permanent, full-time position. If you’re passionate about delivering quality end-of-life care and thrive in a collaborative environment, we’d love to connect with you!</p>

<p><strong>Schedule:</strong><br />
• Monday–Friday, 8:00 AM – 5:00 PM<br />
• 1 evening on-call per week<br />
• Alternating weekends<br />
• Caseload: ~22 patients<br />
• Start: ASAP</p>

<p><strong>Salary:</strong> $75,000–$80,000 annually</p>

<p><strong>Requirements:</strong><br />
• Active Texas RN License<br />
• BLS Certification<br />
• 2–3 years of hospice case management experience<br />
• Experience with WellSky EMR<br />
• Knowledge of HOPE Medicare requirements (preferred)<br />
