Executive Overview
We are seeking a compassionate, experienced Director of Nursing / Clinical Director to lead and elevate hospice clinical operations in the Canyon Lake area.
This is a hands-on executive leadership role within an early-stage, mission-driven hospice program. The DON will play a critical role in building clinical infrastructure, shaping team culture, and ensuring excellence in end-of-life care delivery.
This position is ideal for a leader who thrives in building programs, mentoring clinicians, and providing direct patient care while developing scalable systems.
Core Responsibilities
Clinical Leadership (Startup Phase)
- Serve as an RN case manager (admissions, routine, and urgent visits)
- Deliver hands-on hospice care across home, ALF, SNF, and hospital settings
- Participate in weekday on-call coverage with structured clinical support
Leadership & Team Development
- Lead, mentor, and support RN, LVN, and hospice aide teams
- Drive clinical excellence, compliance, and patient-centered care delivery
- Provide real-time clinical decision-making and field support
Program & Quality Development
- Participate in interdisciplinary team (IDT) meetings
- Develop and refine clinical workflows, standards, and best practices
- Partner with leadership to build and scale a community-based hospice program
Leadership Growth Path
- Phase 1: 70–80% field-based clinical leadership and patient care
- Phase 2: Transition into 70–80% leadership/oversight as census grows
- Opportunity to help define long-term clinical structure and operational model
Ideal Candidate Profile
- Active Texas RN license required
- Strong background in hospice, home health, or acute care leadership
- Proven ability to lead teams in dynamic or startup environments
- Excellent clinical judgment with calm, steady leadership presence
- Passion for end-of-life care and building meaningful patient experiences
- Comfortable working independently in the field
