Position: Internal Medicine Provider
Location: Department of Veterans Affairs North Texas Health Care System, Tyler Centennial VA Clinic, 428 Centennial Pkwy, Tyler, Texas 75703- 7166

Direct Patient Care: estimated 95% in direct patient care.

Schedule:
VA Business Hours: Monday through Friday, 8:00 am to 4:30 pm Central Time.

Qualifications:
License: The Contractor’s physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property.

All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor’s physician(s) who have current, full, and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application will not be considered for the purposes of this contract.

Board Certification: All Contractor’s physician(s) shall be Board Certified or Board Eligible in Family Medicine, Internal Medicine, or a related specialty, certified by the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA) within 4 years of completing training. They must also be currently certified in Basic Life Support (BLS) or their equivalents. All continuing education courses required for maintaining certification must be kept up to date at all times. The Contractor shall provide documentation verifying current certification to the VA Contracting Officer's Representative (COR) on an annual basis for each year of contract performance.

Clinical Services: Contractor physician(s) shall provide comprehensive clinical out- patient services, including but not limited to the following:

Provide independent care delivery for chronic, acute/same day medical needs. This type of care is served to the Veteran population, with diseases consistent with those seen in the general population namely diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, etc. A part of your responsibility is to write orders for diagnostic testing, prescribe medications, consult with specialty service, and utilize computerized/electronic medical records. Major functions of the position include Health risk Assessment, goal setting for prevention, and chronic diseases management. A major focus is on preventive medicine with focus on the achievement of positive outcomes for patients, improving coordination of care, decreasing clinic waits or delays, and appropriate use of resources.

Provide direct patient care to include, but are not limited to history and physicals, medication management, evaluation of diagnostic tests and treatment planning, clinical reminder completion as applicable.

Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Oversee PACTs compliance to Message Manager, MyHealtheVet, and encounter completion. Proficient in Tele-Health to home visits (VA Video Connect), and Comprehensive Women’s Health care.

Document all medical information in electronic medical records.

Perform medication reconciliation.

Other duties as assigned.

???????Clinic Schedule: The Contractor physician(s) will see patients as scheduled. Vesting appointments are scheduled for one hour long, while primary care visits are scheduled for 30 minutes. The Contractor physician may see 9-12 patients per day.
